Telehealth Informed Consent

What is Telehealth?

Telehealth involves the use of electronic communications to deliver healthcare services remotely. LodeRx uses asynchronous telehealth (secure messaging and document exchange) and synchronous telehealth (video or phone consultations when clinically indicated) to provide clinician evaluations and ongoing care.

Consent to Treatment

By proceeding with the intake process, you consent to receive healthcare services via telehealth. You understand that telehealth is not a substitute for in-person care when in-person evaluation is clinically necessary, and that your clinician may refer you to an in-person provider if needed.

Risks and Limitations

Telehealth has limitations including: inability to perform a physical examination, potential technology failures, and the possibility that your clinician may determine telehealth is not appropriate for your condition. In such cases, you will be referred to an in-person provider.

State-Specific Requirements

Telehealth regulations vary by state. Your treating clinician is licensed in your state of residence. Some states require initial synchronous (live) consultations before prescribing. Your clinician will inform you of any state-specific requirements during intake.

Right to Withdraw

You may withdraw consent to telehealth at any time by notifying your clinician through the patient portal or by contacting support@loderx.com.
