HIPAA Notice of Privacy Practices

This Notice Describes

How medical information about you may be used and disclosed and how you can get access to this information. Please review it carefully.

Our Duty

We are required by law to maintain the privacy of your protected health information (PHI), provide you with this notice of our privacy practices, and follow the terms of the notice currently in effect.

Uses and Disclosures

We may use and disclose your PHI for treatment (sharing information with your prescribing clinician and pharmacy), payment (processing your subscription and insurance claims for lab work), and healthcare operations (quality improvement, compliance audits).

We will not use or disclose your PHI for marketing purposes without your written authorization. We do not sell your PHI.

Your Rights

You have the right to: inspect and obtain a copy of your PHI, request amendments to your PHI, receive an accounting of disclosures, request restrictions on uses and disclosures, request confidential communications, and file a complaint if you believe your privacy rights have been violated.

Breach Notification

We will notify you of any breach of your unsecured PHI as required by law, within 60 days of discovery.

If you believe your privacy rights have been violated, you may file a complaint with us at privacy@loderx.com or with the U.S. Department of Health and Human Services Office for Civil Rights. We will not retaliate against you for filing a complaint.
